چکیده
AN INSTANCE of a unique combinationi 1A . of anomnalies was recently encountered in ani inifant with eyanotic congenital heart disease who died at the age of 13 months. The clinical course and pathologic finding,s are described. Case History The patient was a femiiale baby, born at full terim after ani uneventful pregnancy. At birth she weighed 6 lbs. and was eyanotic. Heart disease was first diagnosed at the age of 3 muonths. The par-ents had noted poor weight "gaiin, delayed developmiient, and generally poor health. The family history was noncontributory. At 7 mnoiths sh1e weighed 9 lbs. 7 oz. and could not sit up. Physical examuination revealed muarked cyanosis and elubbinig, perhaps miiore mlarked in the toes. The heart rate was reguhir at 150 per miiinute; the heart was not enlarged. A grade I:l systolic mtiurmtiur was heard in the pulmonary areai with a normial second sounid. The liver was felt two fingers enlarged in the epigastrium. The femioral pulsations were normiial. On fluoroseopy the heart was slightly largel than normal, the pulmionary eonus xwas not remarkable. The upper mediastinal shadow was broad and extended well to the right, suggesting extremile dextroposition of the aiorta. The lung fields were oligemic. In the right aniterior obliquie view the aorta arched to the left. In the left anterior oblique positionI the large supraeardiaic shtadow to the right was confirlmed as a lairge dextroposed aorta. The right ventriele wa,is (dimninutive anid did1 not extend as far forward as the aorta above. The puliiionary window was niot clear and the left venitricle was enlarged. An x-ray confirmiied the fluor-oscopic findings (fig. 1). The liver appea-red transposed with the larger lobe on the left, although with barium the stomach was on the left. Serial electrocardiograms were taken (fig. 2). Examlination of the blood revealed a heml-oglobin of 12 Gmii. per 100 ml. and a hematocrit level of 56 per cent. The white cell count was 7,000 per eu. imii.

Radioisotope Scanning of the Liver and Spleen in Dextrocardia and in Situs Inversus with Levocardia.
KNOWLEDGE of the position of the abdominal organs is important in cardiac diagnosis. Thus a mirror-image dextrocardia is usually observed in a patient with complete situs inversus; this condition more frequently occurs in an otherwise normal individual than in one with a malformed heart. By contrast, when dextrocardia occurs without situs inversus, malformation of the heart is the rule. When th...
متن کاملA Case of Unusual Visceral Heterotaxy Syndrome with Isolated Levocardia
Situs inversus of the abdominal organs in the presence of normally placed heart on the left side of the thorax is known as situs inversus with isolated levocardia. This rare condition is commonly associated with severe congenital defects of the heart. We report a case of situs inversus with levocardia in a 19-year-old asymptomatic male patient with completely normal heart on the left chest. Spi...

متن کاملSitus Inversus - Report Of Twins
Dextrocardia with situs inversus (situs inversus totalis) is rare congenital anomaly which can be asymptomatic and compatible with normal life. Incidence is 1 in 10000. But in cases of situs inversus with levocardia association of congenital heart disease is high. We are presenting a pair of twins, born out of nonconsangunious marriage, from a primigravida mother, with an uneventful antenatal p...

The incidence is about 1:10,000 live people. Normal human development results in an asymmetrical arrangement of the organs within the chest and abdomen. Typically, the heart lies on the left side of the body (levocardia), the liver and spleen lie on the right. Mirror image i.e. transposition of abdominal and thoracic viscera is termed Situs inversus totalis. It has shown the anomalies of rotati...
متن کاملThe Triply Twisted Heart: Cyanosis in an Adult With Situs Inversus, Levocardia, Double Outlet Right Ventricle, and Malposition of the Great Arteries
We present a case of a 19-year-old female presenting with cyanosis since birth. The major anomaly demonstrated was a "triply twisted heart" with a balanced physiology, allowing her to survive into adulthood. Non-invasive imaging was done using 2D and real-time 3D (or 4D) echocardiography with multi-slice imaging from 4D volume datasets. Findings were confirmed using cardiac magnetic resonance i...
